My standard session is 55 minutes. Sessions are billed at a rate of $160 per session. The initial intake session runs for 60 minutes and is billed at a rate of $180. Payment is due at the end of each session and can be made via check, cash, or credit card (Visa, MasterCard, American Express, or Discover).
I am an In Network Provider for Aetna, the First Choice Health Network, Kaiser Permanente Access, Options, and Omni PPO Plans, Lifewise, Premera Blue Cross, Regence BlueShield, and Uniform Medical Plan. I am also able to accept most out of state Blue Cross and Blue Shield plans via the National Blue Card Program.
For other insurance companies I am considered an Out of Network Provider. That means I don't bill your insurance company directly for the services I provide but I can give you a receipt that you can submit to your insurance company for reimbursement. Insurance plans vary so you should check with your insurance company to learn about your coverage for Out of Network Providers.
My Client Information Form, Personal Disclosure Form, and HIPAA Notice of Privacy Practices are included at the links below. Please print them out before our first session. Complete the Client Information Form and then read over the Personal Disclosure Statement and HIPAA Notice of Privacy Practices - you’ll have a chance to ask any questions you may have when we meet. If you’re planning to use insurance to pay for sessions please complete the Insurance Agreement Form as well.
